Thyroid disorders are now among the most common chronic conditions in India - studies suggest that approximately 1 in every 10 Indians lives with some form of thyroid imbalance, and women are nearly 8 times more likely than men to be affected. In urban centres like Ahmedabad, sedentary lifestyles, iodine variability, autoimmune disease and rising stress are pushing those numbers even higher. Yet many patients walk around with unexplained fatigue, weight changes or hair fall for years before the diagnosis is finally made with a simple blood test.

At Heal1 Pharmacy in Bopal, we dispense thyroid medicines to thousands of families every month. This guide is our way of helping you understand what the thyroid does, how to spot trouble early, what to eat, and how to take your medicines correctly for the best results.

What is the Thyroid?

The thyroid is a small, butterfly-shaped gland sitting at the front of your neck, just below the Adam's apple. Despite weighing only about 25 grams, it controls your metabolism, body temperature, heart rate, weight, mood and energy levels. It does this by producing two key hormones:

  • T4 (Thyroxine) - the storage hormone
  • T3 (Triiodothyronine) - the active hormone

Production is regulated by TSH (Thyroid Stimulating Hormone) released from the pituitary gland in the brain. When the thyroid produces too little hormone, you have hypothyroidism; when it produces too much, you have hyperthyroidism.

Hypothyroidism (Underactive Thyroid) - Symptoms

This is the most common thyroid problem in India, especially in women aged 30-55. Symptoms often build up slowly:

  • Unexplained weight gain despite eating normally
  • Constant fatigue and low stamina
  • Feeling cold even in warm weather
  • Dry, flaky skin and brittle nails
  • Hair fall, including thinning of eyebrows
  • Constipation and bloating
  • Depression, low mood, poor concentration ("brain fog")
  • Irregular or heavy menstrual periods
  • Puffy face, swollen feet
  • Hoarse voice and slow heart rate

Hyperthyroidism (Overactive Thyroid) - Symptoms

Less common but more dramatic in presentation:

  • Unintentional weight loss despite eating well
  • Anxiety, irritability and restlessness
  • Rapid or irregular heartbeat (palpitations)
  • Excessive sweating and heat intolerance
  • Fine tremors of the hands
  • Insomnia and disturbed sleep
  • Frequent loose motions
  • Bulging eyes (in Graves' disease)
  • Muscle weakness, especially in the thighs

Common Causes

  • Hashimoto's thyroiditis - the leading autoimmune cause of hypothyroidism
  • Graves' disease - autoimmune cause of hyperthyroidism
  • Iodine deficiency or excess
  • Postpartum thyroiditis - after childbirth
  • Thyroid nodules or goitre
  • Side effect of certain medicines like amiodarone or lithium
  • Family history of thyroid or autoimmune disease

Diet for Thyroid Patients - Indian Foods

For Hypothyroidism

  • Use iodized salt in cooking (don't switch to rock salt entirely).
  • Include dairy - milk, curd, paneer - for iodine and selenium.
  • Eat eggs, especially the yolk, which is rich in iodine, selenium and vitamin D.
  • Add fish like rohu, surmai or mackerel 2-3 times a week if non-vegetarian.
  • Snack on Brazil nuts, almonds, walnuts and pumpkin seeds for selenium and zinc.
  • Eat berries, oranges, guava and amla for antioxidants.
  • Limit raw cabbage, cauliflower and soy - cooking deactivates the goitrogens, so cooked is fine in normal amounts.

For Hyperthyroidism

  • Include cruciferous vegetables (cabbage, broccoli, cauliflower) which mildly slow thyroid hormone production.
  • Boost calcium and vitamin D with milk, ragi, sesame seeds and sun exposure - high thyroid hormones weaken bones.
  • Avoid excess iodine - go easy on seaweed, kelp and high-iodine supplements.
  • Cut caffeine - tea, coffee and colas worsen palpitations and anxiety.

Common Thyroid Medicines

  • For Hypothyroidism (Levothyroxine): Thyronorm (Abbott), Eltroxin (GSK), Letrox (Berlin Chemie), Thyrox (Macleods). These are synthetic T4 hormones, taken lifelong in most cases.
  • For Hyperthyroidism: Carbimazole (Neo-Mercazole), Methimazole, Propylthiouracil (PTU). Beta blockers like Propranolol may be added to control palpitations.

Important Tips for Taking Thyroid Medicine

  • Take on an empty stomach, first thing in the morning, with plain water.
  • Wait at least 30-60 minutes before tea, coffee, milk or breakfast.
  • Take at the same time every day for steady hormone levels.
  • Maintain a 4-hour gap from calcium, iron, antacids or multivitamins - they reduce absorption.
  • Never stop the medicine on your own, even if you feel better.
  • Store in a cool, dry place; keep away from direct sunlight.

Regular Testing

Once your dose is stable, get a TSH test every 6-12 months. After any dose change, retest in 6-8 weeks. Pregnancy, weight changes of over 5 kg, or new symptoms warrant earlier testing. T3 and T4 are added when results are unclear or in hyperthyroidism follow-up.

Lifestyle Tips

  • Practice yoga - Sarvangasana, Matsyasana and Ujjayi pranayama support thyroid health.
  • Manage stress with meditation; chronic stress worsens autoimmune thyroid disease.
  • Walk or exercise 30-45 minutes daily.
  • Sleep 7-8 hours - poor sleep raises TSH.
  • Quit smoking - it worsens both hypo and hyperthyroidism.

Highly processed foods, excess soy products, raw cruciferous vegetables in very large amounts, and refined sugar. Avoid taking your thyroid pill with milk or calcium.
